In consideration of being permitted to participate in any way in Somerset Wheelmen-sponsored bicycling activities: I do hereby, for myself, for my heirs, executors, and administrators waive, release, and forever discharge any and all rights and claims for property damage and/or personal injury which I may have or which may occur to me, against the Somerset Wheelmen members or any of their officers, members sponsors, agents or representatives, arising out of traveling to, participating in, or returning from any of these activities.

I promise to abide by the Constitution and By-Laws of the Somerset Wheelmen and such other rules and regulations as may be adopted. I apply for membership in good faith and intend to race under the Somerset Wheelmen colors for the entire year. I will represent Somerset Wheelmen during competition with fair play and good sportsmanship. I will use the name "Team Somerset" when representing Somerset Wheelmen in all USA Cycling events. If I wish to terminate my membership before the end of the period, the club may refuse to release me.

Your signature indicates you have read, understand and agree to all of the conditions set forth in the By-laws and listed above. All Adult members must sign release; parent or guardian signature required for all members under age 18.

Parent/Guardian Waiver for Minors (under 18 years old)

The below-signed parent and natural guardian or legal guardian does hereby represent that he/she is, in fact, acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parent or legal guardian.
